Not listed?
If your carrier is not on the list
If your insurance has out-of-network coverage, please confirm it with your plan before you make an appointment. We do not accept Medicaid plans, or other plans with no out-of-network coverage. Without out-of-network coverage, your visit is self-pay.
Not sure where you stand? Call or text our front desk at (904) 686-7771 and the team will help you before you book.
Aesthetics and wellness
Aesthetic care is self-pay; wellness coverage varies
Aesthetic treatments such as Botox, lasers, peels and skin tightening are self-pay, with payment required at the time of service.
Insurance coverage for wellness treatments such as weight loss, hormone therapy and Exomind varies depending on your plan and the indication for treatment. Our team will verify your benefits and explore payment options with you.
We accept major credit cards, CareCredit and Cherry payment plans. Cherry lets you split treatment costs into more manageable monthly payments, with a quick online application. See cost and payment options.
At your visit
What to bring and what you will pay
Bring a valid photo ID and your insurance card or cards. We recommend contacting your insurance provider in advance to confirm that our office is in-network. Your appointment request will be declined if your insurance information is not complete.
Payment for services is due at the time of your visit and may include a copay, coinsurance or any unmet deductible, depending on your plan. A credit card is kept on file for payment, and cash is not accepted. Read our new patient information for everything else to prepare.
