How booking works

Send a request below and our front desk will call or text you to confirm your appointment.
For a medical visit, you will then receive an email from noreply@ezderm.com to set up your patient portal, where you can complete your intake forms and consents before you arrive, so your visit starts on time. Prefer to book by phone? Call or text (904) 686-7771 and we will take care of it.
Before a medical visit
Have your insurance card ready unless you are paying for yourself, because we need complete insurance details to confirm a medical appointment. We recommend checking with your insurer that our office is in network. If your plan has out-of-network coverage, please confirm it before your visit.
To keep wait times short, complete your check-in through the patient portal before you arrive. Once you are logged in, your intake questions and consent forms are under the Appointments tab.
- Bring a valid photo ID and your insurance cards
- Arrive with makeup removed and no lotion or moisturizer applied
- Wear comfortable clothing that is easy to change in and out of
- Bring your current skincare products so they can be reviewed
Payment and appointment policies
Payment is due at the time of your visit and may include a copay, coinsurance or any unmet deductible. Aesthetic services are self-pay. A credit card is kept on file for payment, and we do not accept cash.
If you need to cancel or reschedule, please give us at least 24 hours notice. Appointments missed without notice may be subject to a no-show fee, except in extenuating circumstances. Patients who arrive more than 10 minutes late may be asked to reschedule.

Book a medical visit for skin checks, skin cancer, rashes, acne, eczema, psoriasis or another skin condition. Book an aesthetic consultation for injectables, lasers, skin rejuvenation or hair restoration, and a wellness consult for weight loss, hormones, peptides or recovery. If you are not sure, call or text and our team will help.
For medical dermatology we accept Aetna, Blue Cross Blue Shield of Florida and Medicare. We do not accept Medicaid plans. Aesthetic and wellness treatments are self-pay.
