We DO accept:
- Medicare: Medicare Part B, Railroad Medicare
- Blue Cross Blue Shield of Florida (BCBS FL): Medicare Advantage PPO (MAPPO), Medicare Supplemental (MDS), Individual Blue Options (NWB), Blue Choice/PPO (PPC), Payment for Professional Services (PPS)
- Cigna: PPO, HMO, OAB, Local Plus
- Aetna: all plans
- Self-Pay: required at the time of cosmetic services or if no out-of-network insurance coverage
- Out-Of-Network Coverage: If your insurance has out-of-network coverage, please confirm with your plan prior to making an appointment.
We DO NOT accept:
- Any Medicaid plans
- Other insurance plans with no out-of-network coverage

How to Prepare For Your In-Office Visit
Complete Patient Paperwork on our Portal
Our standard in-office check-in is completed on an iPad, but to minimize wait times and help us maintain a limited lobby capacity, we strongly encourage patients to complete their check-in through the patient portal before their appointment. The portal also gives you convenient access to important visit details, including your appointment date and time, diagnoses, counseling notes, and treatment recommendations.
To get started, please verify your email using the message sent from noreply@ezderm.com after booking your appointment, then create your username and password to sign in. Once logged in, complete all required medical intake questions and sign your consent forms, which can be found under the “Appointments” tab at the top right of the screen. For your examination
Please arrive at your appointment with all makeup removed and avoid applying any lotion or moisturizer beforehand. Wear comfortable clothing that is easy to change in and out of for your exam, and be sure to bring your current skincare products with you so they can be reviewed during your visit.
Insurance and Payment
Please bring a valid photo ID and your insurance card(s) to your appointment, and we recommend contacting your insurance provider in advance to confirm that our office is in-network. Payment for services rendered is due at the time of your visit and may include a copay, coinsurance, and/or any unmet deductible based on your plan benefits. A credit card is required to be kept on file for payment, and please note that cash payments are not accepted.
Office Hours
Clinic hours are Monday through Friday from 9:30 AM–11:30 AM and 1:30 PM–3:30 PM. The lobby is closed daily from 12:00 PM–1:00 PM.
No Show and Late Appointment Policies
No-Show Policy: If you need to cancel or reschedule your appointment, please notify us at least 24 hours in advance. Appointments missed without proper notice may be subject to a no-show fee, except in extenuating circumstances.
Late Appointment Policy: Patients arriving more than 10 minutes after their scheduled appointment time may be asked to reschedule.
